_______ Please accept my entry in the state qualifier for the Junior World Championship (San Diego, CA), which is being held at the Tumwater Valley Golf Club in Tumwater on Friday, June 29, 2007. I understand that I must first play in my sub-district qualifier for this event to earn entry into the state qualifier on June 29, at Tumwater Valley Golf Club. I understand that if I earn an opportunity to play in the national event I must have my parent's permission to give tournament officials my answer as to my intentions prior to the beginning of play on the day of qualifying. Parent's signature: _____________________________________________________ ENTRIES MUST BE RECEIVED IN THE OFFICE NO LATER THAN Thursday, May 31st, 2007. NOTES: 1. Entry fees are NOT REFUNDABLE; so be sure you are playing in the sub-district event designated as JR World Qualifier before entering. 2. Late entries are accepted on an individual basis only. 3. If you are 14 you do not have to qualify to play in the JR WORLD in San Diego, but you may still enter this event to play for points. 14 year olds can enter the Junior World Championship directly by calling (619) 280-8505 and requesting an entry form.
